Used the wrong lock ring set to hold the bearings in place.

Well... 4 years ago when I replaced the wheel bearings and upgraded to manual locking hubs I didn't install the Mile Marker lock nut kit. Re-using the factory, keyed washer and lock ring didn't work out so well.
