Can anyone help me with the identification of my tranny?

Hi All,
I bought this truck about 2 months ago, and we have since done the whole tranny seal change (what a weekend that was.. ), but since then we have come to realize that this is not the original transmission that came in this truck, because this one has band adjustments. Is there anyone here that knows anything about matching the numbers off of the transfer case? Unfortunately, these are the only numbers we have to go on, and the transmission is leaking again which leads me to believe that it is not the right seal because that was definitely the problem originally.
I had the truck for a week, it drove wonderfully, and then when the hubby drove it, I get a phone call saying it won't go into gear. I go to the parking lot where my poor truck is sitting only to find out that the seal has "broken" and my baby truck has bled out all over the ground ( i added this section to preface why we changed the seal originally). Anyway, obviously we need to change out the seal yet again, but at least it is not massively losing fluid. It takes about a quart a week depending on how hard I drive it (which at times can be pretty hard.. ). Anyway, enough babbling, and any help would be greatly appreciated.

P.S. We think it might be an al4d, but we aren't positive.
