88 Crown Vic -Vacuum Plumbing Help Needed

This engine is out of an '88 Crown Vic LTD, but is mounted in a hotrod. I have not installed the air pump / ACV or ABV system, but have kept the EGR valve and solenoid. I keep getting an error code 34 - EGR Valve Malfunction. I've tried installing a new EGR Solenoid, but still get the error code. I'm beginning to think there's an issue with the way I've routed the vacuum lines.

Can anyone tell me if the vacuum lines for the EGR valve need to go to a different port on the intake manifold then the fuel pressure regulator vacuum line? The Emission Control Information sticker (E8AE-9C485-AAD) shows what appears to be 4 ports off of the intake mainfold - 1 for the MAP, one for the PCV, one going to EVR/ACV/ABV solenoids and the last going to a CPRV (assuming this is the fule pressure regulator). Can you help me identify what port is what on the manifold?

Any other suggestions?
