Here is the crux of the failure, see the rounded off tips of the engagement pins? They are supposed to be cylindrically shaped with sharp edges. When the locker failed at Tellico it was under heavy throttle and the locker made a ratcheting sound, I'm certain that is when the brunt of this damage must have occurred.
